I hereby voluntarily apply to become a member of the Organization of SMUD Employees (hereinafter “Union”) and agree to abide by the Union’s Bylaws and membership policies. I hereby authorize SMUD to deduct from my salary or wages the amount requested by the Union for membership dues, initiation fees, general assessments, and payments for membership benefit programs sponsored by the Union for which I have applied, and to remit the total amount deducted to the Union on my behalf. This authorization shall remain in effect for a minimum of 12 months and may only be cancelled by me annually thereafter during the month of May. I understand that any notice of cancellation of this application and authorization must be submitted to the Union in writing. I understand that the Union will thereafter advise SMUD of my cancellation of this application and authorization, and that SMUD will then take reasonable efforts to discontinue further deductions during the first full pay period following the month of May. I understand that cancellation of this application and authorization will cause me to forego all rights and benefits enjoyed exclusively by Union members.
